*Please note that any change in transportation must be phoned in or emailed to the school secretary before 2:30 p.m. daily.

The streets and parking lots around the school are extremely busy at certain times during the school day. It is important for the safety of the children that they listen to and respect the directions given to them by the school staff.
Due to the amount of traffic at the school we ask that all parents be aware of the proper traffic patterns when dropping off a child. RCSD busses ONLY will enter the main traffic loop between both Summit Schools at general drop off & dismissal time between RHS-Summit Campus & Summit Elementary.
Students being picked up/dropped off by vehicles should enter the south main parking lot at the front entrance of Summit Elementary (Door #1) , maintain the lane until the final turn, then pull as far forward in line as possible. Display the name of the child(ren) you are picking up in the passenger window on the sign provided by the school. If you need additional car pick up signs, please contact the school office.
Unless you are picking your child up early from school, please do not park your vehicle in the visitor parking spaces near the front entrance doors during dismissal time. This will help prevent congestion during dismissal time. Your understanding is deeply appreciated.

It is very important that your child's school has most current contact information for any school/district communications or in case of an emergency.
ALL RETURNING STUDENTS will need to have their information reviewed and/or updated by completing an Emergency Authorization Form at the beginning of each new school year.

Parent may also update and review student information electronically through the PowerSchool Parent Portal.

Additional Forms
The following forms are available for your convenience.
Emergency Authorization Form - this form is required by ALL STUDENTS each new school year to review/update contact and medical information. In addition, this updated form is required in order for a student to participate in any school field trips.
Residing with Friends or Family Packet - this packet is required to be completed each year to establish residency. These families consist of those living with others who own the home in Reynoldsburg other than themselves. If this packet is not completed prior to the first day of school it could affect the students enrollment by placing them on the Summit STEM waiting list.
Prescription Medication at School Form - These two forms need to be completed by the parent and doctor before a student can begin taking medication at school. In addition, our policy is that prescription medication must be delivered personally to the school in its "original" container in which it is dispensed.
Non-Prescription Medication at School Form - This form needs to be completed by the parent before a student can begin taking the medicine at school. In addition, this request must be delivered to the school in its original container.
Parent Note to School - this form can be used to verify a student's absence upon their return to school. Otherwise, a personal note/email from the parent/guardian will also do.
Pre-Authorized Planned Absence Form - this form must be filled out PRIOR to any planned absences that will last for three (3) or more school days. First extended absence - up to 30 consecutive hours will be excused if form is turned in prior to absences.

Annual Notice: Parents Bill of Rights
The Ohio legislature has enacted the Parents’ Bill of Rights that affirms your fundamental right as a parent or guardian to make decisions concerning the upbringing, education, and care of your child. The District has adopted Board Policy [5780.01/IGBLA/other], which describes the specific rights you have as a parent or guardian and requires the District to notify you regarding these rights and to ask you to choose whether the District can provide certain health care services to your child. The District offers the following physical, mental, and behavioral health care services to your child:
- School-based behavioral health services (by referral only): Nationwide Children’s Hospital, The Buckeye Ranch, Integrated Services Behavioral Health, Southeast Healthcare, Inc. (Prevention services)
- School Social Workers, School Nurses, Health Aides
The District is authorized by law to employ health care service professionals and to provide certain services – among them, diabetes care, conducting hearing and vision tests, testing for tuberculosis (if required by the Board of Health), screening for newly enrolling kindergarten or first grade students, dyslexia screenings, medication administration, concussion assessments, seizure action planning, threat assessments, counseling to any victim of sexual harassment or sexually related conduct, counseling for habitually absent students, and any other services required by a student’s Individualized Education Plan or 504 Plan.
Other options are available for your child to receive physical, mental, and behavioral health services. You can see your primary health care provider. Programs, including a public health clinic, are also offered by Nationwide Children’s Hospital. The District has also compiled a list of counseling and behavioral health resources available on reyn.org under the Department of Diverse Learners & Whole Child Supports. The link can be found here.
To facilitate parental involvement and decision-making, you have the option to withhold consent or decline any service offered by the District. If you consent, it does not waive your right to access your child’s educational or health records or to be notified about a change in your child’s services or monitoring as required by law.
Note: This notice and consent does not apply to emergency situations, first aid, other unanticipated minor health care services, or health care services provided pursuant to a student’s IEP or 504 Plan. If you have questions about these exceptions, please contact the District office.
